Birth or Baptismal Certificate
- required for each dependant child if you are a single parent, married, or living common-law;
- must contain the names of both parents (where applicable);

Canada Child Tax Benefit Confirmation
- required if you are a single parent in order to confirm receipt of Canada Child Tax Benefits;
- can be obtained by calling the Canada Revenue Agency at 1-800-387-1193;

Common-law Documentation
- you must submit a letter from an unbiased, unrelated third party (e.g. doctor, teacher) who has personal knowledge of your living arrangements and can confirm the date of cohabitation; (Confirmation from your landlord is not acceptable.)
the third party must submit their contact information, occupation and their relationship to you;
- affidavits are not acceptable;
- if you cannot obtain a third-party letter, you can submit copies of your tax returns which document that the relationship has been in place for at least 12 months. If your relationship began on January 1 of the calendar year, you are only required to submit the tax return for that year. Otherwise, two consecutive tax years are required.

Daycare Confirmation
- if you have a child(ren) 11 and under listed as a dependant on your application, you must provide a letter from Human Resources Labour and Employment (HRLE) and/or the Department of Health and Community Services (HCS) verifying you are not receiving a daycare subsidy from either provincial department;
- your application will be assessed without a daycare cost and will be reassessed once this verification is received;

Disabled Dependants
- if your disabled dependant(s) is over the age of 12 years, you must submit a letter from a physician confirming that daily care is required;
- receipts verifying the amount claimed must be submitted;

Extended Youth Care Agreement
- if you are in receipt of funding from the Department of Health and community Services, via the Youth Services Agreement, you must provide a letter to confirm the amount of assistance;

Information/Signature of One Parent
- information from both biological parents is required. However, information from one parent may be accepted in legitimate exceptional circumstances where there is contact with only one parent;
- submit a written explanation from the parent with whom you live;

Prior Semester/Academic Year
- if you have been placed on a restriction code for not successfully completing either 60 percent of a full course load for the Canada Student Loans or 80 percent of a full course load for Newfoundland and Labrador Student Loans in a prior semester, you will be required to submit confirmation of successful completion of each subsequent semester in order to be approved for additional student financial assistance;
