- Student’s Declaration, Consent and Signature Form

- Required to be completed by all students. Fax, mail or drop off the form at
our office at Coughlan College.
- Canada Revenue Agency Signature Form

- As part of the Student Financial Services Division’s audit and verification
process, you, the applicant, and where applicable, your parent(s) or spouse
must sign this form and submit to the Student Financial Services Division.
Mail or drop off the form at our office at Coughlan College. Fax copies are
not acceptable.
- Income Confirmation Form

- Required to confirm the income you earned during the pre-study period. Must
be submitted by the deadline date (i.e., eight (8) weeks after the start of
classes) to receive your Up-front Provincial Study Grant, Canada Study Grant
and/or your Millennium Bursary (if applicable), and/or your next semester’s
loans. Fax, mail or drop off the form at our office at Coughlan College.
